Why is one part of my zap broken but matches my other zaps that seem to be working fine

I have a zap made for one of my online fitness programs. 

The step in the workflow where the client is to be added to my PT Distinction app, is saying “app returning 400” when I try to test it. 

My other zaps for 2 other programs are running fine and I don’t see any differences in the information I have entered for them. Does anyone know why this may be happening?
