Teamwork CRM error: The app returned "Duplicated Key Error".

Hello dear support,
I have the following problem when searching for a contact:
For example, zapier send the following data

But by searching zapier get a completely incorrect contact.
His name and email do not match:

 

 

And then when zapier try to create a contact zapier get an error stating that such a contact already exists.
The app returned "Duplicated Key Error".
Please help me solve this problem.

Before we dig deeper into this, would you mind sharing a detailed screenshot of how your Zap is configured? Also, please share a screenshot of the “Action” section of the “Find Contact” action step that shows all of its fields. Like so:
