How do I set up a Zap to add website opt-ins to corresponding Salesforce campaigns based on selected checkboxes?

I have an opt-in page on my website where you can opt-in to be notified of upcoming classes. There are 6 checkboxes for the different types of classes we offer. 

 

When someone opts in, I need to add them to a corresponding campaign in Salesforce. How would I be able to do this in Zapier? I believe filters will stop it altogether if it doesn’t match the first filter. Do I need a loop after a path? How do I ensure it doesn’t keep adding to the same campaign over and over?
