I have a very simple zap that just gets a trigger from a form submission and then adds the data to Attentive. I’ve tested it and it works but when i toggle it on, refresh the page, it’s toggled off again. no error no history

I did some digging on my end, and the issue here might be your API permissions from Gravity Forms the API key may have not have write permissions.
To fix this, could you kindly check the permissions for the API you used for the Gravity Forms connection? Here's a help doc from Gravity Forms to help you set it up: https://docs.gravityforms.com/rest-api-v2-authentication/#authentication-credentials.
