Umbraco Forms error: Unable to load choices We're having trouble loading 'Form' data. The app returned "Server Error"

I am trying to add a Form in Umbraco to Zapier and no forms are appearing and I get the message:

Unable to load choices We're having trouble loading 'Form' data. The app returned "Server Error".

Hi @PEDuncan

This looks like an error from the app side, you can retry later and that doesn’t work I would suggest you to contact the Umbraco Forms support.
