Trouble sending Squarespace form data to ConvertKit

I have a zap that is supposed take info from a contact form on our squarespace site and then send that info to our Kit account.

It tests fine and sends exactly what it is supposed to, to exactly it is supposed to.

However, when anyone fills out the form it doesn’t appear in convert kit.

Can you help with this

I wasn’t able to get it to work, so I went with another solution.
