Trigger Does not cause action outside of Zapier

When I work on my zap within zapier, it triggers as intended. When I complete the trigger outside of the program (submitting a survey), it does not action at all.

What am I doing wrong?

Jotform Zap triggers are scheduled, meaning it can take from 1-15 minutes to fire depending on your Zap plan.

Turn your Zap ON and test, then check your Zap Runs history details for errors and to see the DATA IN/OUT for each Zap step:
