Salesforce connection to Smartsheet

not all entries from Salesforce are coming over into Smartsheet 

I am not getting an error of any sort, they are just not coming over. 

Thoughts on trouble shooting

Do you mind clarifying your current workflow in terms of triggers and actions? Sometimes it helps providing screenshots of your trigger and action setups as well!
