Error selecting value in Google Forms: How to troubleshoot?

why am getting this error message when I want to select the value for form and how do I fix it?

Before we dig deeper into this, have you tried creating a Form in Google Forms? After that, please click on the “Refresh results” button and see if it’ll load the form that you have created.

I'll be keeping an eye out for your response!
