I have set-up the following Zap:

 

 

The test with dummy information works but then when I do a test with my own email & name it shows an error.

Check your Zap Runs history details to see the DATA IN/OUT for each step to help you troubleshoot: https://zapier.com/app/history/

For Zap 2, instead use this Zap action: MailerLite - Create or Update Subscriber

You can set the desired Group.
