So, apparently I managed to set up my zap but there is an issue I can’t find the solution to exactly the format with which Zapier adds the response on a new card.

Am I correct that the screenshot you provided is the content that came across in your Trello card from Google Forms? One thing to try would be to make sure your Google Form responses are saved to a Google Sheet in order to work with Zapier. You can read more here!
If that doesn’t seem to be the culprit, you can share a shot of your Zap configuration with our Support team via this link and they can troubleshoot from there!
