Integrating phone App and CRM system

I will be using 3 platforms - a phone, a CRM system and Zapier. I want to use Zapier to facilitate caller ID when someone rings us, whereby Zapier would pull the person’s name from our CRM system using the phone number from the phone platform. Your assistance would be much appreciated. The CRM system is Jobadder.com and I have enabled Zapier as an Integrated Add-on.

Thanks. I had tried something like that (Trigger event was when a Record was Changed/Updated, and Action was to Create a Note), but all the Trigger events that could be selected had to originate from Jobadder, whereas I think I need the Trigger to be from the Phone App (www.nuacom.ie) which then Actions an event from Jobadder (i.e. it passes the Caller ID from Jobadder to Nuacom) ?
