I need to send a questionnaire to my clients. This form will ask unique questions to each person, so it will need to be zapped from an excel/google sheet and made into a one-time only use form. I’ve looked everywhere I could but can’t find an app on Zapier that accommodates this. Anyone have any ideas?

I think you are going to be hard pressed to find such an app, let alone if such an app does exist, getting it configured to work with Zaps.
Are you not able to standardize the questions asked to recipients?
I'm using standardized questions now and it gets the job done. I just have to manually separate and enter the information. If I could do what I wanted, it would all zap where I need it. I think I'm getting spoiled 😆

Automation is really predicated on having standardized data.
Freeform text and lots of logic exceptions isn’t ideal for trying to automate.
