How to connect Formidable Forms and Docage. When a Formidable Forms entry is created I nedd to automate a Docage signature request

Hello,
Do you know how I can make the link between Formidable Forms and Docage. I would like that when a Formidable Forms form is created, it sends a Docage signature request directly to the mailbox of the person who filled all the information in the form

Hi Troy ! Thank you for your answer. 

I have tried the steps you have indicated. Nevertheless I have a problem, I have this error message that appears: The app returned "Guid should contain 32 digits with 4 dashes (xxxxxxxx-xxxx-xxxx-xxxx-xxxxxxxxxxxx).".

Do you have a solution?

@hicham 

You need to select the Transaction Template ID from the dropdown instead of mapping the variable.

Remove the mapped variable, then look at the list of the dropdown options.

Notice the smaller gray font value that is expected for each friendly name option.

I would love to but unfortunately the list of the dropdown options is empty.
