How do I upload a new document here? It is showing an option from May, i dont remember how I did it previously. I have selected TRIGGER - NEW SIGNED DOCUMENT, maybe last time i used SUBMITTED document thats how i made it work.

Good question.

New documents would need to be uploaded IN Jotform.

In the Zap action you can map a dynamic variable from the Zap trigger step using the ‘Custom’ tab.
