How do I create an onboarding form to import Salesforce into a custom object?

Hi everybody,

In my organization we would like to create an onboarding form that has to be filled-in by clients and then easily imported in Salesforce.

This data would be imported on a custom object.

I don’t want to convert my PDF to a CSV and use Import Wizard every time. 

Do you know any app that could help me with that?

If you use an esign app then all the data would be digital and structured for us in automation between the apps.
