How do I create a new entry in monday.com when someone downloads a Gumroad product?

Hello,
 

I give away worksheets and digital resources on my gumroad account. When someone downloads any of my resources on gumroad, I would like to create an entry in monday.com with their name, email, phone #, and have a sub item added with the resource they downloaded. 

HELP!!!
