How do I automate client information and stage updates in FUB using a Jotform form?

I am currently trying to set up my jap form form to trigger contact information update in. Follow up Boss. I am stuck at the point where you test the zap. I’m getting errors and not sure how to fix this. If anyone has any experience with this please Let me know!

 

i’m trying to automate the process of getting clients’ information into their profile and changing there stage in FUB via a Jotform form.
