Create a more attractive pdf using data from a PDF or Microsoft Excel file

When a customer completes a form, we get a pdf/excel format document export, however the layout of the document is poor.

 

I am looking for a way to take the data from either the pdf or excel file and add it to a clearer laid out pdf file.

 

Does anybody have any experience with this or had to do something similair?
