There are a few ways you could tackle this, but in the end what you’re looking for is a lookup table.
You can use the Lookup Table in our Formatter app, but if you’re looking to re-use this across Zaps then I’d suggest something like Google Sheets, Excel, or Airtable. You could also check out our very own Zapier Tables (currently in beta).
The concept will be the same regardless of which option you choose. Make a table much like what you showed in the screenshot, add a step to your Zap that searches the table (make sure that what your trigger provides matches what’s in your table) then in your CRM step you map the result from your search :)
This example should give you a good idea of what I’m suggesting:
