Webhook not finding form

I am new to Zapier and I have started a Zap with a Ninja From submission as the trigger. The webhook has been added to the form and the form has been published. BUT When testing the Zap the form is not found. It is telling me to make sure Ninja Forms has the Zapier Add-On, Which it already does and I confirmed it is active. Why is the form not being found?
