Why are the data fields in the sent email blank? See screenshot below.

As you can see in the screenshot below the data populated correctly for the Zap when setting up the Zap and the test email was sent. 

 

However, in all subsequent emails the data fields have not been populated. 

 

Please help.

Check your Zap Runs history details to see the DATA IN/OUT for each step to help you troubleshoot if the data was present at all: https://zapier.com/app/history/
