If you still need help, post screenshots showing how your Zap steps are configured in EDIT mode with the field mappings visible in the CONFIGURE tab, plus screenshots showing the DATA OUT from the Zap trigger step.

If so, we’d love to hear what the solution was - this would be so helpful for folks that might be getting a similar error. 
And if not, can you share some screenshots as Troy mentioned so we can take a closer look? Remember to use a tool like Zappy to remove any personal info like names, emails etc. before sharing as this is a public space.

Yes I did sort it with a work around after also speaking with ScoreApp.
It turns out the API pushes the data from ScoreApp as soon as the quiz is completed and that does not include any forms at the end. So the quiz data was coming through but not the form data.
What I had to do was change the trigger from Quiz completed to Lead Signed Up and keep the rest of my filters relating to the quiz. The other workaround suggested was to move the form to the front of the quiz but we felt that would be a barrier for our clients and we wouldn’t get the same return.
This worked and I have raised a feature request with both ScoreApp and Zapier to have this amended as I know most people will also have a form at the end.
