THis is NOT an inconsitency problem.
This was working fine for months, and now it has stopped working. When I run a test, it works fine, it finds the recent leads just fine.
It is published and just no longer triggering. I have tried recreating the trigger from scratch. I have tried re-creating the zap… It has just stopped triggering.
How do I get it to trigger again?

Have there been any changes to the lead form at all? If a Facebook Lead Ads form is updated it’ll have a new ID so if the Zap is still looking at the old ID then that might be why it’s no longer triggering. If that’s the case here then you’d need to edit the Zap to reselect the form then republish it.
If it's not that then I'd suggest double checking the connected account still has the necessary permissions, as something may have changed there. Pulling a test record doesn't need the full set of permissions, which may explain why you're seeing new leads as test records but not getting live triggers.
