Linkedin forms are not visible while creating zap for linkedin form ads. Does anyone know if I am missing anything?

Welcome to the Community. 

 

In order to troubleshoot issues with LinkedIn Lead Gen Forms in Zapier, ensure you have an active Lead Gen Form linked to an active ad campaign. Check that Zapier is connected to your LinkedIn account by reviewing "Connected Accounts" in Zapier, and refresh the LinkedIn connection if needed. Verify that your LinkedIn account has the necessary permissions (Account Manager or higher) to access Lead Gen Forms.

Did Jammer’s suggestion help to get you pointed in the right direction? 

If not, do you happen to know the ID for the form that’s missing? Just thinking that you might be able to select it manually if you use the Custom value option for the Lead Form field then type in the ID of the form. Could you give that a try and let us know whether that allows you to select the desired form?
