Google Docs error: The requested conversion is not supported

Keep getting an error that says “The requested conversion is not supported.” when trying to test a Zap that is supposed to create a Google doc from a Google form submission. Cannot figure out what the issue is. Anyone have any ideas?

Please post detailed screenshots with how your Zap step is configured along with the encountered error.
