Formatter with utilities not returning output from lookup table

I have a simple zap where I am taking the output of a google form and pushing the details into a Jira new issue.

I am trying to use Formatter - Utilities to do a lookup table and maps the Jira ID for the selection to the name of the selection in Jira:

But when I test it, it doesn’t pull back the output at all:

What am I doing wrong?

Switch the data sides.

e.g. she/here should be on the left and 10288 should be on the right
