Failed to create a Lead in Salesforce: checklist: value not of required type

I am getting an error message in one of my forms saying that it cannot create a Lead in Salesforce because a checkbox field is “not of required type”.  Even when I set a default value of false to the field, I am getting the same error message. 

Has anyone else encountered this?  How did you remedy it?
