Error message:[ALERT] Possible error on your Ext 101 Zap!

This is one of my first zap I created and every so often I get this message from various user I have.   Each of my user have been assigned an extension.  The zap is to phone recording...I can’t seem to get help support and I believe I have it right but not sure as the error is random.  Sometimes it can be to Ext 107 or 103 and so forth.   I am having the recording saved in Onedrive if that helps.

To help us have context, please post detailed screenshots with how your Zap steps are configured along with an example of the encountered error.
