DocuSign fields missing data

I’m having the same issue. I have 1 sender, recipient’s name and phone are filled out, the docusign labels are unique, the docusign fields are not required and I am getting the docusign envelope returned with missing data.

Before we dig deeper into this, would you mind sharing a detailed screenshot of how your Zap is configured? Also, please share a screenshot of the “Action” section of the action step that shows all of its fields. Like so:

Did you manage to resolve things here? If so we’d love it if you could share details of what the solution was.
