ConvertKit Error: Unexpected status code 404 from https://api.convertkit.com/v3/forms/0/subscribe

Im having issues connecting my Squarespace newsletter form to converkit via zapier. I keep getting an error code:  

Failed to create a subscribe to form in ConvertKit

Error from halted execution: Unexpected status code 404 from https://api.convertkit.com/v3/forms/0/subscribe

 

I have tried to follow along and trouble shoit and Ive been at it form hours, folliwing all sort of instructions and just cant seem to get it to work. Any advice possible would be helpful.

Please post screenshots with how your Zap steps are configured to give us more context.

And post a screenshot of the encountered error.
