Why would a zap not run

I have a zap set up. The test goes through, I just had a lead come through, but the zap did not run. I ran another test and it came through. I am not sure what the problem might be.

    For us to have context, post screenshots with how your Zap steps are outlined and configured in EDIT mode.

    How is that configured in the origin app? (screenshots)
