Question

I want to send one WPForms [feedback form] via email or text if another one has a specific box ticked

I have just created a PIN request form which has a hidden checkbox field (for admin use only) and I tick yes or no.

If I tick yes then I want the trigger to send an email containing a feedback form to that recipient.

Try adding a Filter as Step 2 in the Zap: https://zapier.com/apps/filter/help
