How to split up form values all contained in one field

How can I extract values and put them in their own fields?

As you can see above, some form fields (your name, your email and best number to call) were all joined together and sent in “Task Description” field that the zap sees. 

I need to use the values (Mary Smith, mary@xyz.com)

How can I split them into their own?

Try using the Formatter > Text > Split action in your Zap: https://zapier.com/help/doc/how-use-formatter-functions
