How to Create a project in modcapcutapk with a template?

I'm trying to set up a Zap such that, when completing a Google form, a new project with a template is created on ModCapCutapk.com. However, I can't seem to find an option to apply the template. 

I'm able to create the project, but I'm having trouble finding information about automatically applying a template. If you could help, it would be greatly appreciated.
