How do I import a CSV file to create records in a Zapier Table

my clients will fill in a form sent from Constant Contact and I can export this CSV:

I want to write a record to a Zapier Table, and I can rearrange the columns, but if I try to import a properly-formatted CSV file, it says it’s “not a valid file”

This is the error.

I’m trying a few different approaches.

I created a Form in Zapier that has input boxes for the info I need, but I don’t know how to send it out.

I want the trigger to be

When a client fills out the form and submits it, write the info into a row in a Google Sheets spreadsheet.

Some options:

 

You can use a GForm and link it to a GSheet, no Zap needed.
