How do I format dates and phone numbers from a Google Form to a Google Sheet?

Question

I am trying to get information from one spreadsheet (that’s automatically completed as google forms responses come in but we hate the format). i.e. the date/time stamp when a simple 4/22 is what we are looking for. I can format the initial sheet but it doesn’t stay formatted for new forms responses. Is there a way to format the zap to change the date and phone number formats that come in a different way?
