What app is beings used for the submission sheet? (e.g. GSheets)

I have integrated Google sheets. I have tried this look up feature and it does give me multiple responses but I am not able to store it anywhere. Like, is it possible to store the multiple rows data in an object or something?

I want data to create some sort of report, so I am using pdf monkey for the same, to create the report and send the report over mail.
