I’m trying to set up the following automation and I’m stuck at a few steps. I’d really appreciate any help or guidance 🙏

Goal:

When someone submits a form on my website via WPForms, I want to:

  1. Automatically populate a DocuSign template with the data submitted through the form,

  2. Then send the filled document to the form submitter’s email address for signature.

Current Setup:

  • Trigger: WPForms (new form submission)

  • Action: DocuSign – “Create Signature Request from Template”

  • I’ve selected the correct Template ID.

  • In the Role/Recipient section, I’m using the form submitter’s name and email address.

  • In the Field Mapping step, I’m matching the form fields to the corresponding fields in the DocuSign template.

If anyone has experience setting up a similar workflow or has suggestions for best practices, I’d be grateful for your input.

Thanks in advance!

Hi @mozsoy05

Help us have more info by detailing what issues/errors you are having with configuring the Zap steps.

Post screenshots showing how your Zap steps are outlined and configured in EDIT mode with the field mappings visible.

I shot a video for you, all the steps are visible in the video. My problem is that the information from the form does not fill in my template agreement in docusign. It needs to be filled in and sent by e-mail.

Hi @mozsoy05,

To troubleshoot issues with your Zap sending data to Docusign, confirm that the trigger correctly captures form data and that each field is accurately mapped to the Docusign template. Use Zapier's test feature to verify data flow, review Zap history for errors, and ensure your Docusign connection is authenticated. Lastly, double-check the email step to confirm the completed document is being sent to the intended recipient.

I did my tests but I did not receive the pdf contract by email.

@mozsoy05

If you want to fill and send a Docusign Envelope from a Docusign Template, then use this Zap action: Docusign - Send Envelope Using Template

TIP: Make sure to add clear data labels to your Docusign Template: https://support.docusign.com/s/document-item?language=en_US&bundleId=gmi1660583110357&topicId=yis1665092718418.html&_LANG=enus

FYI: For your email step, the attachment field expects a file object or a file url, rather than plain text ID values.

Hi @mozsoy05,

Just following up here, did Troy’s suggestion do the trick for you?

We’re more than happy to help if you need further support. 🤗

Unfortunately it didn't work I guess I'm doing something wrong. I know this is not right but can you support me via anydesk?

Also there is no data label section in the docusign template section.

Hi @mozsoy05,

You're on the right track with your Docusign integration in Zapier. The missing attachment in the "Email by Zapier" step is due to the Docusign action not returning a downloadable file, so you'll need to either call the Docusign API using a Webhooks step to retrieve the completed document or let Docusign handle the email notification directly. Additionally, template fields aren't appearing for mapping. In that case, it's likely because those fields don't have "Data Labels" set—adding clear, unique labels in the template and refreshing the Zap setup should resolve that.
