When a Google form is filled out, it is entered on a Google sheet. I see that I can create a zap to add a task to Teamwork each time a new entry is added to the sheet.

Is it possible to add the task to different projects in Teamwork or does it need to be the same project each time?

If you can see the Project selection field in the Teamwork>Add a task action then you can definitely set up rules on which projects to go where.

For easier selection you can use Paths where you can use filters and based on your google form data you can route tasks in different projects.

For advanced system (where you have more than 4 projects) you can use “lookup table” or a code step (Javascript/Python) to route task to different projects based on the input.
