Hi, can you please help me what could be the problem or how can I fix it?

Error message: “Failed to create a request in Webhooks by Zapier
could not execute statement; SQL [n/a]; constraint [account_unique_per_partner]; nested exception is org.hibernate.exception.ConstraintViolationException: could not execute statement (HTTP Status Code: 400)”
