How can I create a Zap to check value in one of two WPForms fields and send to Pipedrive?

Hello! I have a form from WPforms and we are sending the data to Pipedrive.

The form has a select field to choose age (children or adult). Based on the choice, I display another select field (using wpforms conditional logic) to display another options. So I have two selects that has the address that should be sent to pipedrive.

Is it possible to create a zap that checks which select field has value and send this value to pipedrive?
