Facebook Lead Ads Zap is not triggering for new leads

So, i’ve paid for Zapier premium to be able to use this service. 

However, leads are being generated by Facebook, but the Zap doesn’t do anything.

While my ad was running, 16 leads should have triggered the zap. 

I’ve now turned it off.

Why is this?

Usually, it’s to do with permissions issues in your Facebook account.
