No responses are added to Google Sheets when testing leads from Google Ads

Hi want to connect my Google Ads Lead collection form with Google Sheets through Zapier, but unable to do the same. I have tested the triggers multiple times and there is no response in the sheet. 

Can anyone suggest how to integrate the lead form with Google Sheets?
